Connected Care Model

Managing your health can be challenging, especially if you have two or more chronic illnesses. Medicare is offering you, in partnership with your doctor, a new program that will help you manage your health in between visits with your doctor. If you have Medicare and two or more chronic conditions, talk to us about chronic care management services and how you might be able to get connected care.
